P-Cincher: A modified band contouring plier

  Abstract 


This article presents modification of band contouring plier into arch wire cinching plier (P-Cincher), which is easy to modify and user-friendly. The conventional method for arch wire bending is by heating and bending of distal ends of the arch wire with the help of light wire plier or Weingart plier, this method of arch wire bending is time-consuming, and it may loosen bands or debonding of bondable buccal tube while cinching. This plier is designed to overcome disadvantages of conventional method, and it will definitely help in faster and effective cinching of arch wire.

  Introduction Top


Introducing nickel–titanium (NiTi) alloys has made a revolution in orthodontic wires industry. Since NiTi wires were first supplied in the orthodontic market, over 30 years ago, improvements have been obtained to enhance its performance in dental leveling and alignment.[1] Buehler has observed the shape memory effect, asserting that, in low temperatures, the NiTi alloys were easily deformed and when heating them above their transition temperatures, their original configurations were reestablished.[2],[3]

NiTi wires are widely used within orthodontics for the leveling and alignment as they combine shape memory effect and superelasticity with excellent corrosion and mechanical propertie, and good biocompatibility. Cinching of NiTi wire is one of the problems among orthodontists and pediatric orthodontists, due to its property of shape memory and even heating of NiTi wire is troublesome and time-consuming. In routine practice, loosening of bands or deboning of buccal tubes is observed while bending of NiTi wires. Patient compliance and comfort is of paramount importance in any orthodontic treatment.[4] Many patient complaints of pricking and hurting of orthodontic wire, especially with NiTi wire if it is not cinched properly.


  Procedure Top


To overcome the disadvantages of conventional cinching, a modification in plier is designed, which will help in faster and effective cinching of arch wire. A band contouring plier is modified into arch wire cinching plier (P-Cincher), which is a simple, easy, and user-friendly procedure to get a precise and efficient plier to cinch NiTi as well as stainless steel arch wires.


  Technique Top


  1. A band contouring plier (worn-out or condemned) is selected [Figure 1]
  2. The round beak of the plier is trimmed with disk on both the sides and made into a triangular shape and pointed [Figure 2]
  3. Concave shape beak is recontoured to modified pointed beak [Figure 3]
  4. The plier design is made such that satisfactory arch wire bend is formed [Figure 4]
  5. Place the NiTi or stainless steel wire (rectangular or round) with approximately 1 cm excess distal to molar tube and hold the wire with P-Cincher plier and press.

Advantage of P-Cincher plier over conventional techniques

  1. It is economical as it is modification of worn-out or condemned band contouring plier
  2. Band contouring plier can be easily modified into P-Cincher [Figure 5]
  3. Quick and effective cinching of both round and rectangular NiTi and stainless steel wires
  4. Amount of loosening of bands and deboning of bondable tubes, while cinching will be avoided
  5. Heating of the distal wire is followed in conventional method, whereas it is not needed with P-Cincher, as this plier helps in cinching wire without heating and bending, is faster and effective.

  Conclusion Top


The modified plier (P-Cincher) is a simple and user-friendly, which is used to bend both round and rectangular NiTi and stainless steel wires. This method of cinching is less time-consuming and effective compare to the conventional method cinching of arch wires.
